Child’s name and date of birth
Parent/carer name

Support areas (e.g. communication, feeding)
Preferred session days and locations
Referral source (e.g. doctor, teacher, family/friend, self-referral)

This information is collected solely to assess suitability for speech pathology services and assist in therapy planning.

2. How We Use This Information

The information provided is used to:

Determine whether services are suitable for your child’s needs
Facilitate the scheduling of therapy sessions
Create a client profile if therapy services proceed
Contact you regarding services

If your child does not proceed to therapy, all submitted information will be deleted promptly.

3. How We Store Your Information

Form submissions are collected through our website (built on Wix) and, if services proceed, transferred securely to Cliniko, a practice management system. Only Kea Mason (sole practitioner) has access to this data.

Data is not shared with any third parties without your explicit consent, unless required by law.

You have the right to:

Access the information we hold about you or your child
Request corrections
Request deletion of your information
